According to Cinnaholic's 2025 Franchise Disclosure Document, prospective franchisees are required to complete a Franchisee Disclosure Questionnaire. This questionnaire aims to identify any unauthorized or misleading statements made during the franchise sales process and to ensure the franchisee understands the limitations on potential claims related to their franchise purchase and operation. If a prospective Cinnaholic franchisee answers 'No' to any question on the Franchisee Disclosure Questionnaire, they must explain their answer on the back of the questionnaire sheet. This explanation provides Cinnaholic with additional context and details regarding the franchisee's response.

The Franchisee Disclosure Questionnaire must be signed and dated the same day the Development Agreement and/or Franchise Agreement is signed and when the development and/or franchise fee is paid. However, it cannot be signed or dated on the same day as the Receipt for the Franchise Disclosure Document. This timing ensures that the franchisee has had adequate time to review the FDD before committing to the franchise.
