As you know, Cajun Global LLC ("we", "us", "our") and you are preparing to enter into a Franchise Agreement for the operation of a franchised Church's Texas Chicken restaurant or a Development Agreement for the development of Church's Texas Chicken restaurants. The purpose of this Questionnaire is to determine whether any statements or promises were made to you that we have not authorized and that may be untrue, inaccurate or misleading. Please review each of the following questions carefully and provide honest and complete responses to each question. This Questionnaire is not applicable to and shall not be used with respect to any franchise offer and/or sale involving a Maryland resident and/or if the franchised restaurant is to be located in Maryland.

What This Means (2025 FDD)
The 2025 Churchs Chicken Franchise Disclosure Document includes a Compliance Questionnaire for new franchisees/developers. The document states that the purpose of the questionnaire is to determine if any unauthorized statements or promises were made to the applicant that may be untrue, inaccurate, or misleading.
The questionnaire includes a series of questions (10-15) asking whether any employee or person speaking on behalf of Churchs Chicken made certain statements or promises regarding potential earnings, revenue, costs, success, advertising, marketing, training, support, service, assistance, or agreements concerning the purchase of the franchise that contradict the information in the FDD.
The FDD states that this document will not be signed by you, and will not apply, if the offer or sale of the franchise is subject to the state franchise registration/disclosure laws in the states of California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin. The questionnaire is also not applicable to and shall not be used with respect to any franchise offer and/or sale involving a Maryland resident and/or if the franchised restaurant is to be located in Maryland.
