| "Franchise"). The purpose of this Summary of Acknowledgments and Franchise Questionnaire is | |
| to determine whether any statements or promises were made to you that Franchisor has not | |
| authorized and that may be untrue, inaccurate or misleading. |

What This Means (2025 FDD)
According to the 2025 Bumper Man FDD, the franchise agreement includes a Franchisee Questionnaire as Exhibit H. However, this questionnaire is not used if the franchise is to be operated in, or the franchisee is a resident of, California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin.
The purpose of the Bumper Man Summary of Acknowledgments and Franchisee Questionnaire is to determine whether any unauthorized statements or promises were made to the franchisee that may be untrue, inaccurate, or misleading.
It is important to note that in certain states, any questionnaire signed by the franchisee cannot waive claims under state franchise law or disclaim reliance on statements made by Bumper Man or its representatives.
