FRANCHISEE DISCLOSURE QUESTIONNAIRE AND CERTIFICATION
Please complete the attached questionnaire as you work through the process of becoming a 1-800-GOT-JUNK? franchisee. When you have completed the questionnaire, please sign the certification that appears on the last page.
The overall purpose of the information collected by this questionnaire is to determine whether any statements or promises were made to you by employees or representatives of 1-800-GOT-JUNK? LLC that 1-800-GOT-JUNK? LLC has not authorized, and that may be untrue, inaccurate or misleading.
In addition, questions relating to statements made to you during the application process, you will also find questions relating to the dates that certain documents (such as the Franchise Disclosure Document or Franchise Agreement) were received, or dates on which payments were made.
When purchasing a franchise, the timing of the receipt of documents, payments of franchise fees and other events are very important.
Did your receive a copy of Company’s Franchise Disclosure Document at least 14 calendar days prior to signing the 1-800-GOT-JUNK? Agreement or making any payment to Company?
Have you received and personally reviewed the 1-800-GOT-JUNK? Agreement and each exhibit, addendum and schedule attached to it?
Have you discussed the benefits and risks of operating the franchised business with an attorney, accountant or other professional advisor and do you understand those risks?
Has any employee or other person speaking on our behalf made any statement or promise concerning the revenues, profits or operating costs of the franchised business that we or our franchisees operate?
This Franchisee Acknowledgement is not applicable to and shall not be used as to any franchise offer and/or sale involving any California resident and/or franchisee as the Franchisee Acknowledgement violates of California Corporations Code sections 31512 and 31512.1.
Do not sign this Questionnaire and Certification if you are a resident of Maryland, or the business is to be operated in Maryland.
Do not sign this Franchisee Disclosure Questionnaire and Certification if you are a resident of Washington, or the business is to be operated in Washington.

What This Means (2025 FDD)
According to the 2025 1-800-GOT-JUNK? Franchise Disclosure Document, a prospective franchisee must complete and submit the Franchise Disclosure Questionnaire and Certification at the time they sign the 1-800-GOT-JUNK? Agreement. This questionnaire is designed to determine if any unauthorized, untrue, inaccurate, or misleading statements or promises were made by 1-800-GOT-JUNK?'s employees or representatives during the franchise application process. The document also gathers information about the dates certain documents were received, and payments were made.
The Franchise Disclosure Questionnaire includes questions about whether the franchisee received the Franchise Disclosure Document at least 14 calendar days prior to signing the 1-800-GOT-JUNK? Agreement or making any payment to the company. It also asks whether the franchisee has received and personally reviewed the 1-800-GOT-JUNK? Agreement and all associated exhibits, addenda, and schedules, and whether they understand all the information contained within those documents. The questionnaire further addresses general considerations, such as whether the franchisee has discussed the benefits and risks of operating the franchised business with an advisor and understands those risks.
The questionnaire also covers communications with 1-800-GOT-JUNK?, inquiring whether any employee or representative made statements or promises about revenues, profits, operating costs, or the likelihood of success that contradict the information in the Disclosure Document. The franchisee must certify that their answers are truthful, as 1-800-GOT-JUNK? will rely on them. The completed Franchise Disclosure Questionnaire, along with the 1-800-GOT-JUNK? Agreement, should be sent to 1-800-GOT-JUNK?'s headquarters in Vancouver, B.C., Canada.
It is important to note that the Franchisee Acknowledgement is not applicable to California residents or franchisees, as it violates California Corporations Code sections 31512 and 31512.1. Additionally, residents of Maryland or Washington, or businesses to be operated in those states, should not sign the Questionnaire and Certification.
