You will provide us, on execution, with an undated assignment of the telephone number to us, in the form annexed hereto as Attachment 5;

You shall require and obtain execution of covenants similar to those set forth in this Section 19.4 (including covenants applicable upon the termination of a person's employment with you) from all employees, contractors or third persons who will have access to Mr. Sandless proprietary and Confidential Information, and shall provide to us to an executed versions thereof. Such covenants shall be substantially in the form set forth in Attachment 4 as revised and updated from time to time and contained in the Manual.

What This Means (2025 FDD)
According to the 2025 Mr. Sandless Franchise Disclosure Document, franchisees must provide an undated assignment of their telephone number to Mr. Sandless upon execution of the franchise agreement. This assignment must be in the form annexed as Attachment 5.
Additionally, if the franchisee is a corporation or limited liability company, they must furnish corporate or limited liability company books and records, including minutes of meetings, to Mr. Sandless upon request. The articles of incorporation and bylaws of a corporate franchisee, or the operating agreement of a limited liability company, must reflect the Franchise Agreement and all other agreements specified by Mr. Sandless. The franchisee must also submit to Mr. Sandless any documents relating to the corporation or limited liability company as Mr. Sandless may require.
Furthermore, Mr. Sandless requires franchisees to obtain and provide executed versions of covenants, similar to those outlined in Section 19.4 of the franchise agreement, from all employees, contractors, or third parties who have access to Mr. Sandless's proprietary and confidential information. These covenants should be substantially in the form set forth in Attachment 4 and as revised and updated from time to time and contained in the Manual.
