As a prospective Learningrx franchisee, what am I required to provide when completing the New Franchisee Disclosure Questionnaire?

As you know, you and LearningRx Franchise Corporation are preparing to enter into a Franchise Agreement for the operation of a LearningRx franchise. This questionnaire cannot be signed and dated the same day as the Acknowledgment for the Franchise Disclosure Document.
If you are a franchisee in any of the following states, please do not answer Questions 4 or 11 – 15, as such questions are not applicable to you: California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin
Do not sign this Questionnaire if you are a resident of California, Maryland or Washington, or your business will be operated in the states of California, Maryland or Washington.
All Representations requiring prospective franchisees to assent to a release, estoppel or waiver of liability are not intended to nor shall they act as a release, estoppel or waiver of any liability incurred under the Maryland Franchise Registration and Disclosure Law
This Questionnaire does not waive any liability LearningRx may have under the Washington Franchise Investment Protection Act, RCW 19.100, and the rules adopted thereunder.

What This Means (2025 FDD)
According to Learningrx's 2025 Franchise Disclosure Document, as a prospective franchisee, you are required to review each question on the New Franchisee Disclosure Questionnaire carefully and provide honest responses to each question. If you answer "No" to any of the questions, you must explain your answer on the back of the sheet. The questionnaire is included as Attachment VII to the Franchise Agreement.
Notably, the questionnaire cannot be signed and dated on the same day as the Acknowledgment for the Franchise Disclosure Document. Furthermore, franchisees in California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin are instructed not to answer Questions 4 or 11–15, as those questions are not applicable in those states. Residents of California, Maryland, or Washington, or those whose businesses will operate in those states, should not sign the questionnaire at all.
The FDD also stipulates that any representations requiring prospective franchisees to assent to a release, estoppel, or waiver of liability do not act as a release, estoppel, or waiver of any liability incurred under the Maryland Franchise Registration and Disclosure Law. Similarly, the questionnaire does not waive any liability Learningrx may have under the Washington Franchise Investment Protection Act, RCW 19.100, and its associated rules.
