What This Means (2024 FDD)
According to Kidokinetics' 2024 Franchise Disclosure Document, signing the receipt is an acknowledgement that you signed a receipt for the Franchise Disclosure Document (FDD) indicating the date you received it. This is confirmed in the Franchise Disclosure Questionnaire, which includes a question asking if you signed a receipt for the FDD, indicating the date you received it. You must answer "yes" or "no" to this question.
This questionnaire is not to be used for any franchise sale in or to residents of California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin.
Kidokinetics requires that you do not sign or date this questionnaire the same day as the Receipt for the Franchise Disclosure Document, but you must sign and date it the same day you sign the Franchise Agreement. This questionnaire is designed to ensure that prospective franchisees have received, reviewed, and understood the FDD and Franchise Agreement before making a commitment to invest in a Kidokinetics franchise.
