As you know, Kidokinetics Franchise LLC ("we" or "us"), and you are preparing to enter into a Franchise Agreement for the operation of a Kidokinetics franchise. You cannot sign or date this questionnaire the same day as the Receipt for the Franchise Disclosure Document, but you must sign and date it the same day you sign the Franchise Agreement. Please review each of the following questions carefully and provide honest responses to each question. If you answer "No" to any of the questions below, please explain your answer in the table provided below.
Do not sign this Questionnaire if you are a resident of Maryland or if the franchise is to be operated in Maryland.

What This Means (2024 FDD)
According to Kidokinetics's 2024 Franchise Disclosure Document, when entering into a Franchise Agreement, prospective franchisees must review each question in the Franchise Disclosure Questionnaire carefully and provide honest responses. If a franchisee answers "No" to any of the questions, they must explain their answer in the provided table.
Kidokinetics relies on the answers provided in the questionnaire to ensure the franchise sale complies with state and federal laws. By signing the questionnaire, the franchisee represents that they have considered each question carefully and responded truthfully.
However, the FDD also notes that for franchise applicants in California, certain questions (7, 8, and 15) from the Franchise Disclosure Questionnaire in Exhibit F are deleted and should not be completed. If a California applicant answers these questions, Kidokinetics will disregard the answers and will not rely on any representations inferred from those answers. This highlights the importance of carefully reviewing all addenda and state-specific modifications to the standard franchise documents.
