TO HEALTHSOURCE CHIROPRACTIC, LLC:

| ("HealthSource") clinic in the area set forth on the attached map (the "Protected |
| Territory"), has been approved, subject to entering into a Franchise Agreement and paying the Initial |
| Franchise Fee. To continue the process of obtaining a franchise to operate a HealthSource Chiropractic |
| clinic in the Protected Territory, I am submitting this Franchise Deposit Acknowledgement (this |
| "Acknowledgement") and my deposit in the amount of $ (the "Deposit") to HealthSource. |
| Upon receipt of this signed Acknowledgement and the Deposit, HealthSource agrees to not grant the |

What This Means (2025 FDD)
According to Healthsource Chiropractic's 2025 Franchise Disclosure Document, after a potential franchisee submits a deposit, Healthsource Chiropractic agrees not to grant the protected territory to any third party until after a specified deadline date. In addition to this, Healthsource Chiropractic will provide access to certain training, information, and materials related to operating a Healthsource Chiropractic clinic.
Healthsource Chiropractic may also disclose confidential and proprietary information after the deposit is made. This information includes details about services and products, sales and profit performance of franchises, sources of products, advertising and promotional programs, image and decor, methods, techniques, formats, specifications, procedures, information, systems, knowledge, experience in development, operation, and franchising, and the selection and methods of training employees. This confidential information may be disclosed verbally, visually through training programs, or in written form via the Healthsource Chiropractic Operations Manual.
Furthermore, Healthsource Chiropractic may provide digital or hard copies of marketing materials through its intranet, potentially charging for these copies at their discretion. It is important for prospective franchisees to understand that while Healthsource Chiropractic will provide these resources, the franchisee is responsible for maintaining the confidentiality of the information and using it solely for the operation of their Healthsource Chiropractic franchise.
