We began offering HealthSource Chiropractic franchises in February 2006. Our principal business address is P.O. Box 770050 Lakewood, OH 44107. We operate under our legal entity name, HealthSource Chiropractic, LLC, the name "HealthSource Chiropractic," and no other name. See Exhibit A for our agents for service of process.
