Apex was formed in Ohio on June 2, 2017 and offers a membership program through a discount medical plan organization where participants receive discounts at participating chiropractic clinics, including HealthSource Clinics. Apex has not offered or sold, and does not currently offer or sell, the HealthSource Chiropractic franchises described in this Disclosure Document or franchises in any other line of business, but reserves the right to do so in the future. Apex does not operate any businesses of the type being franchised by HealthSource Chiropractic.
