Our affiliate, Apex Longevity LLC ("Apex"), is an Ohio limited liability company that has the same principal business address as us, 5 Avon Lake, OH 44012. Apex was formed in Ohio on June 2, 2017 and offers a membership program through a discount medical plan organization where participants receive discounts at participating chiropractic clinics, including HealthSource Clinics. Apex has not offered or sold, and does not currently offer or sell, the HealthSource Chiropractic franchises described in this Disclosure Document or franchises in any other line of business, but reserves the right to do so in the future. Apex does not operate any businesses of the type being franchised by HealthSource Chiropractic.

What This Means (2025 FDD)
According to the 2025 HealthSource Chiropractic FDD, Apex Longevity LLC, an affiliate of HealthSource Chiropractic, has not offered or sold HealthSource Chiropractic franchises. Apex Longevity LLC was formed in Ohio on June 2, 2017, and offers a membership program through a discount medical plan organization where participants receive discounts at participating chiropractic clinics, including HealthSource Clinics.
Specifically, Apex has not offered or sold HealthSource Chiropractic franchises described in the FDD, nor has it offered or sold franchises in any other line of business. However, Apex Longevity LLC reserves the right to offer or sell HealthSource Chiropractic franchises in the future.
It is important to note that Apex Longevity LLC does not operate any businesses of the type being franchised by HealthSource Chiropractic. This means that while they may offer discounts for services at HealthSource Clinics, they do not directly manage or operate any HealthSource Chiropractic clinics themselves.
