FOR PROSPECTIVE FRANCHISEES THAT RESIDE IN OR ARE SEEKING TO OPERATE THE FRANCHISED BUSINESS IN ANY REGULATED STATE, SUCH PROSPECTIVE FRANCHISEE IS NOT REQUIRED TO COMPLETE THIS QUESTIONNAIRE OR TO RESPOND TO ANY OF THE QUESTIONS CONTAINED IN THIS QUESTIONNAIRE.
As you know, Hot Chikn Kitchn LLC ("we" or "us"), and you are preparing to enter into a Franchise Agreement and Area Development Agreement, if applicable, for the operation of a HCK Hot Chicken franchise. The purpose of this questionnaire is to determine whether any statements or promises were made to you that we have not authorized or that may be untrue, inaccurate, or misleading, to be certain that you have been properly represented in this transaction, and to be certain that you understand the limitations on claims you may make by reason of the purchase and operation of your franchise.

What This Means (2025 FDD)
According to the 2025 Hck Hot Chicken Franchise Disclosure Document, the purpose of the Franchise Disclosure Questionnaire is to ensure prospective franchisees understand the limitations on claims they can make regarding the purchase and operation of their franchise. Hck Hot Chicken uses the questionnaire to determine if any unauthorized, untrue, inaccurate, or misleading statements or promises were made to the prospective franchisee.
The questionnaire aims to confirm that prospective Hck Hot Chicken franchisees have been properly represented during the franchise transaction. By completing the questionnaire, franchisees acknowledge their understanding of the restrictions on claims they can make about their franchise.
However, the FDD states that if a prospective franchisee resides in or is seeking to operate the franchised business in any regulated state, they are not required to complete the questionnaire or respond to any of its questions. The document also specifies that the questionnaire cannot be signed or dated on the same day as the Receipt for the Franchise Disclosure Document, but it must be signed and dated on the same day as the Franchise Agreement and Area Development Agreement, if applicable.
