FOR PROSPECTIVE FRANCHISEES THAT RESIDE IN OR ARE SEEKING TO OPERATE THE FRANCHISED BUSINESS IN ANY REGULATED STATE, SUCH PROSPECTIVE FRANCHISEE IS NOT REQUIRED TO COMPLETE THIS QUESTIONNAIRE OR TO RESPOND TO ANY OF THE QUESTIONS CONTAINED IN THIS QUESTIONNAIRE.
As you know, Hot Chikn Kitchn LLC ("we" or "us"), and you are preparing to enter into a Franchise Agreement and Area Development Agreement, if applicable, for the operation of a HCK Hot Chicken franchise. The purpose of this questionnaire is to determine whether any statements or promises were made to you that we have not authorized or that may be untrue, inaccurate, or misleading, to be certain that you have been properly represented in this transaction, and to be certain that you understand the limitations on claims you may make by reason of the purchase and operation of your franchise. You cannot sign or date this questionnaire the same day as the Receipt for the Franchise Disclosure Document, but you must sign and date it the same day you sign the Franchise Agreement and Area Development Agreement, if applicable. Please review each of the following questions carefully and provide honest responses to each question. If you answer "No" to any of the questions below, please explain your answer in the table provided below.

What This Means (2025 FDD)
According to Hck Hot Chicken's 2025 Franchise Disclosure Document, after receiving the document, a prospective franchisee is required to carefully read the disclosure document and all agreements. Hck Hot Chicken must provide the disclosure document to the prospective franchisee within a specific timeframe before any binding agreement is signed or any payment is made. This timeframe is generally 14 calendar days, but it varies by state.
In Iowa, the disclosure document must be provided at the earlier of the first personal meeting or 14 calendar days before signing a binding agreement or making a payment. Michigan requires the document to be provided at least 10 business days before the execution of any binding agreement or payment of any consideration, whichever comes first. New York has a similar requirement, mandating the document at the earlier of the first personal meeting or 10 business days before the execution of the agreement or payment of any consideration.
Additionally, the Hck Hot Chicken FDD includes a questionnaire that the prospective franchisee must complete. However, this questionnaire cannot be signed or dated on the same day as the receipt for the Franchise Disclosure Document. It must be signed and dated on the same day the Franchise Agreement and Area Development Agreement (if applicable) are signed. This questionnaire aims to ensure that the franchisee was not subject to unauthorized or misleading statements during the franchise sales process.
