*The following language applies only to transactions governed by the California Franchise Investment Law – This questionnaire does not apply to franchises who intend to operate the franchised business in the State of California.

As you know, FocalPoint Coaching, Inc. and you are preparing to enter into a Unit Franchise Agreement for the operation of a FOCALPOINTTM franchise. In this questionnaire, FocalPoint Coaching, Inc. will be referred to as "we" or "us." The purpose of this questionnaire is to determine whether any statements or promises were made to you that we did not authorize and that may inaccurate. Please review each of the following questions carefully and provide honest and complete responses to each question.

According to Focalpoint Coaching's 2025 Franchise Disclosure Document, the purpose of the Franchisee Disclosure Questionnaire is to determine whether any unauthorized or inaccurate statements or promises were made to the prospective franchisee during the franchise sales process. Focalpoint Coaching uses the questionnaire to verify that the information provided to the franchisee was accurate and authorized by the company.
