*The following language applies only to transactions governed by the California Franchise Investment Law – This questionnaire does not apply to franchises who intend to operate the franchised business in the State of California.
As you know, FocalPoint Coaching, Inc. and you are preparing to enter into a Unit Franchise Agreement for the operation of a FOCALPOINTTM franchise. In this questionnaire, FocalPoint Coaching, Inc. will be referred to as "we" or "us." The purpose of this questionnaire is to determine whether any statements or promises were made to you that we did not authorize and that may inaccurate. Please review each of the following questions carefully and provide honest and complete responses to each question.

What This Means (2025 FDD)
According to the 2025 Focalpoint Coaching Franchise Disclosure Document, the purpose of the Franchisee Disclosure Questionnaire is for Focalpoint Coaching to determine if any unauthorized or inaccurate statements or promises were made to the prospective franchisee. The franchisee is required to carefully review each question in the questionnaire and provide honest and complete responses.
By signing the questionnaire, the franchisee acknowledges that they understand the importance of their answers, that Focalpoint Coaching will rely on those answers, and that they have responded truthfully to all questions.
It is important to note that the questionnaire does not apply to franchisees who intend to operate the franchised business in California, residents of Hawaii, franchises located in Hawaii, residents of Maryland, franchises to be operated in Maryland, residents of Washington, or franchises to be operated in Washington. If the recipient of the questionnaire is a corporation, partnership, or other entity, each of its principals must complete and sign the acknowledgment, making additional copies as necessary.
