If Fat Shack Inc. ("Fat Shack") does not deliver this Disclosure Document on time or if it contains a false or misleading statement, or a material omission, a violation of federal law and state law may have occurred and should be reported to the Federal Trade Commission, Washington, DC 20580, and the appropriate state agency identified on Attachment M.
Fat Shack authorizes the parties identified on Attachment M to receive service of process for it in the particular state.

Listed below is the name and last known city, state and telephone number of every franchisee and area developer who has had an outlet terminated, cancelled, not renewed, or otherwise voluntarily or involuntarily ceased to do business under their respective franchise agreement or area development agreement during the most recently completed fiscal year or who has not communicated with us within 10 weeks of the Issuance Date of this Disclosure Document.
