As you know, Crepe Delicious Holding USA, Inc. (" w e" or " u s"), and you are preparing to enter into a Franchise Agreement for the operation of a Crepe de licious Franchise. You cannot sign or date this questionnaire the same day as the Receipt for the Franchise Disclosure Document, but you must sign and date it the same day you sign the Franchise Agreement. Please review each of the following questions carefully and provide honest responses to each question. If you answer "No" to any of the questions below, please explain your answer in the table provided below.

What This Means (2025 FDD)
According to the 2025 Crepe De Licious Franchise Disclosure Document, a prospective franchisee must receive the FDD at least 14 calendar days before signing a binding agreement or making a payment to Crepe De Licious. This waiting period allows the prospective franchisee adequate time to review the document and seek professional advice. Certain states, such as Iowa, Michigan and New York, have additional requirements regarding when the FDD must be provided. Specifically, Iowa requires that the FDD be provided at the first personal meeting to discuss the franchise. Michigan requires the FDD to be provided at least ten business days before signing any agreement or making any payment, whichever occurs first. New York also requires the FDD to be provided at the earlier of the first personal meeting or ten business days before signing any agreement or making any payment related to the franchise relationship.
To ensure compliance and acknowledgement of the FDD, Crepe De Licious requires the prospective franchisee to complete a Franchise Disclosure Questionnaire. This questionnaire confirms that the franchisee has reviewed the FDD and understands its contents. The franchisee cannot sign or date this questionnaire on the same day as receiving the FDD, but must sign and date it on the same day the Franchise Agreement is signed. This separation in timing reinforces the importance of the review period and ensures the franchisee has had sufficient time to consider the information provided.
Crepe De Licious also uses a Territory Reservation Agreement and Receipt, where a Prospective Franchisee communicates their intent to purchase a Crepe De Licious franchise. A non-refundable Territory Reservation Fee of $10,000 is required to reserve such territory for purchase. The prospective franchisee acknowledges that they have waited the prescribed federal and state waiting periods, but in no event sooner than 14 calendar days after it received Crepe Delicious' Franchise Disclosure Document, before signing this Agreement and paying the Territory Reservation Fee. If the Prospective Franchisee elects not to sign a franchise agreement by the specified date, Crepe De Licious is entitled to retain the entire Territory Reservation Fee.
