To simplify the language in this disclosure document, the terms "Franchisor", or "we" or "us" means CSM Franchise Group, LLC, the Franchisor. The terms "we", "us" and "Franchisor" do not include you, the "Franchisee". We refer to the purchaser(s) of a Crawlspace Medic franchise, as "you" or "Franchisee", whether an individual, a partnership, corporation, or limited liability company. If you are a corporation, partnership or other entity, our Franchise Agreement also will apply to your owners, officers and directors. If you are married and your spouse is not a partner in the franchise business, certain provisions of our Franchise Agreement will also apply to that spouse.
The Franchisor was formed as a Limited Liability Company in the state of South Carolina in December of 2016. Our principal business address is 1049 2nd Avenue, Columbia, South Carolina, 29209, and our telephone number is 888-388-2773. We do business under our company name, "Crawlspace Medic" and its associated design (the "Marks"). Our affiliate, Crawlspace Medic LLC, has registered our primary service marks on the Principal Register of the United States Patent and Trademark Office. We have not offered franchises in any other line of business. We only offer franchises which operate under the "Crawlspace Medic" Marks. We began offering franchises in 2018.
The principal business addresses of our agents for service of process are shown on Exhibit A.
Our Parents, Predecessors and Affiliates
Our direct parent is Home Brands Group, LLC, a South Carolina limited liability company formed on October 1, 2021, and its principal place of business is the same as ours. As of the date of this disclosure document Home Brands Group, LLC does not provide any products or services to our franchisees. We have no other parents or predecessors required to be disclosed in this Item.
We have an affiliated company, Crawlspace Medic LLC, a South Carolina Limited Liability Company with a principal place of business at 1049 2nd Avenue, Columbia, SC 29209. Crawlspace Medic LLC was created in August of 2014 and operates a business similar to the one being franchised. Crawlspace Medic, LLC is the owner of the Marks and has licensed use of the Marks to us.
We have an affiliated company, PLP Franchise Group, LLC formed as a limited liability company in the State of South Carolina in September of 2022 with a principal business address of 1049 2nd Avenue, Columbia, South Carolina, 29209, and our telephone number is 803-753-4326. PLP Franchise Group, LLC only offers "handyman type services" franchises which operate under the "Punch List Pros" Marks.
