As you know, Safer Home Services International, LLC and you are preparing to enter into a Franchise Agreement for the operation of a Clear Pest Pros franchise. Please review each of the following questions carefully and provide honest responses to each question. Do not sign this Questionnaire if you are a resident of Maryland, Washington, or California or the business is to be operated in Maryland, Washington, or California.
