FRANCHISEE DISCLOSURE QUESTIONNAIRE
As you know, CITY PUBLICATIONS FRANCHISE GROUP, INC. and you are preparing to enter into a Franchise Agreement for the operation of a Franchised Business. In this Franchisee Disclosure Questionnaire, CITY PUBLICATIONS FRANCHISE GROUP, INC. will be referred to as "we" or "us." The purpose of this Questionnaire is to determine whether any statements or promises were made to you that we did not authorize and that may be untrue, inaccurate or misleading. Please review each of the following questions carefully and provide honest and complete responses to each question.

What This Means (2025 FDD)
According to City Publications' 2025 Franchise Disclosure Document, Exhibit G is the Franchisee Disclosure Questionnaire. The purpose of this questionnaire is for City Publications to determine if any unauthorized, untrue, inaccurate, or misleading statements or promises were made to the prospective franchisee.
The questionnaire asks a series of yes/no questions regarding discussions with an attorney, accountant, or advisor about the benefits and risks of operating a City Publications business, understanding that the success or failure of the business depends on the franchisee's skills and abilities, competition, economic factors, and lease terms. It also addresses whether any employee or person speaking on behalf of City Publications made statements or promises about revenues, profits, operating costs, or likelihood of success that contradict the information in the disclosure document.
The questionnaire further asks about statements or promises concerning advertising, marketing, training, support services, or assistance that differ from the information in the Disclosure Document. If a prospective franchisee answers "Yes" to questions about unauthorized or contradictory statements, they must provide a full explanation. This process allows City Publications to identify and address any potential misrepresentations or misunderstandings early in the franchise relationship.
