We require that you complete this Questionnaire (a) so that we can determine whether our franchise sales team followed proper sales procedures and (b) to provide us with reasonable assurance that, prior to signing the Franchise Agreement, you have had an adequate opportunity to review the Franchise Disclosure Document and its attachments, consult with legal and/or business advisors of your choosing, and ask us questions about any disclosures or terms that you do not understand. What This Means (2024 FDD)

According to Chop5 Salad Kitchen's 2024 Franchise Disclosure Document, they require franchisees to complete a Franchisee Disclosure Questionnaire before entering into a Franchise Agreement. This questionnaire serves two purposes for Chop5 Salad Kitchen. First, it allows them to assess whether their franchise sales team adhered to proper sales procedures. Second, it provides Chop5 Salad Kitchen with assurance that the prospective franchisee had adequate opportunity to review the Franchise Disclosure Document (FDD) and its attachments, consult with advisors, and ask questions about anything unclear before signing the Franchise Agreement.

Specifically, the franchisee cannot sign the Questionnaire on the same day as they receive the FDD. However, the franchisee must sign and date the Questionnaire on the same day they sign the Franchise Agreement and pay the franchise fee. This timing ensures that the franchisee's final attestation of understanding and voluntary agreement occurs concurrently with their formal commitment to the franchise.

This requirement is a protective measure for both Chop5 Salad Kitchen and the franchisee. It demonstrates that Chop5 Salad Kitchen takes steps to ensure franchisees are informed and prepared, potentially mitigating future disputes. For the franchisee, it serves as a final checkpoint to confirm they have completed their due diligence before making a significant financial investment. Franchisees should take this questionnaire seriously and answer honestly, as it represents their acknowledgement of understanding the terms and conditions of the franchise agreement.
