BACKGROUND AND PURPOSE

The following modifications are made to the CHOP5 Salad Kitchen Franchise Disclosure Document ("FDD" or "Disclosure Document") issued by CHOP5 Franchise LLC ("we" or "us" or "franchisor") to franchisee ("you" or "franchisee") and may supersede certain portions of the Franchise Agreement between you and us dated _________________________, 202__ (the "Franchise Agreement"). What This Means (2024 FDD)

According to Chop5 Salad Kitchen's 2024 Franchise Disclosure Document, the FDD includes state addenda and amendments to the franchise agreement, supplemental agreements, and the FDD itself for certain states. These modifications are made to comply with state laws governing franchise relationships and documents.

The state addendum will modify the agreements to comply with applicable state laws and will only apply if the franchisee meets the requirements of the specific state, independent of signing the addendum. The terms of the state addendum for the applicable state will override any inconsistent provisions in the FDD, franchise agreement, or supplemental documents.

The Chop5 Salad Kitchen FDD specifies that the state addendum applies to California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, and Wisconsin. Prospective franchisees in these states should carefully review the state-specific addenda for any modifications to the standard franchise agreement.
