Answer from 2024 FDD Document

As you know CHOP5 Franchise LLC ("we" or "us), and you are preparing to enter into a Franchise Agreement for the operation of a CHOP5 Salad Kitchen franchise. We require that you complete this Questionnaire (a) so that we can determine whether our franchise sales team followed proper sales procedures and (b) to provide us with reasonable assurance that, prior to signing the Franchise Agreement, you have had an adequate opportunity to review the Franchise Disclosure Document and its attachments, consult with legal and/or business advisors of your choosing, and ask us questions about any disclosures or terms that you do not understand. You cannot sign or date this Questionnaire the same day as the Receipt for the Franchise Disclosure Document but you must sign and date it the same day you sign the Franchise Agreement and pay your franchise fee. Please review each of the following questions carefully and provide honest responses to each question.

What This Means (2024 FDD)

Based on the 2024 Chop5 Salad Kitchen Franchise Disclosure Document, franchisees must sign and date the Franchisee Disclosure Questionnaire the same day they sign the Franchise Agreement and pay the franchise fee. The Franchisee Disclosure Questionnaire includes an ACH (Automated Clearing House) authorization agreement. This means the ACH Agreement must be submitted on the same day the Franchise Agreement is signed and the franchise fee is paid.

This requirement ensures that Chop5 Salad Kitchen has the necessary authorization to process payments electronically from the franchisee's account from the outset of the agreement. This is a fairly standard practice in franchising, as it streamlines the payment process for ongoing fees, royalties, and other charges.

It is important for prospective Chop5 Salad Kitchen franchisees to be aware of this requirement and to ensure they have the ACH Agreement completed and ready to submit on the day they sign the Franchise Agreement. Failure to do so could potentially delay the start of their franchise operations or create administrative complications.
