It also confirms that the franchisee is not relying on any statements, representations, promises, or assurances that are not specifically set forth in the Franchise Disclosure Document and Exhibits when deciding to purchase the franchise.
However, the document states that if the Central Bark Doggy Day Care facility will be located in California, Hawaii, Illinois, Indiana, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin, the franchisee should not sign this questionnaire. Also, if the franchisee is a resident of Maryland or the business is to be operated in Maryland, they should not sign this acknowledgment. Prospective franchisees should confirm with Central Bark Doggy Day Care if there are any other exhibits that should be included in the Disclosure Document.
