As you are aware, you have or are preparing to enter into a Franchise Agreement with C.T. Franchising Systems, Inc. (Franchisor) for the operation of a Caring Transitions® franchise. The purpose of this questionnaire is to determine whether any statements or promises were made to you, either verbally or in writing, that the Franchisor did not authorize and that may be untrue, inaccurate, or misleading. Please read each of the following questions carefully and provide honest and complete responses to each question.

What This Means (2025 FDD)
According to the 2025 Caring Transitions Franchise Disclosure Document, Exhibit O includes a Franchisee Acknowledgment Statement. This statement is designed to determine if any unauthorized promises or statements, either verbal or written, were made to the prospective franchisee by Caring Transitions.
The acknowledgment statement requires the franchisee to confirm that they received the FDD at least 14 days before signing the Franchise Agreement or paying any money for the franchise. It also asks whether the franchisee has read and understood the franchise contracts and their exhibits. If the franchisee does not understand something, they are required to specify what parts they do not understand.
Furthermore, the questionnaire asks if the franchisee has discussed their purchase of a Caring Transitions franchise with an attorney, accountant, or other professional advisor. It emphasizes that the success or failure of the Caring Transitions franchise depends on various factors, including the franchisee's skills and abilities, the number of hours they are willing to work, competition, interest rates, the economy, and other business factors. The franchisee must also acknowledge their responsibility for complying with all applicable laws, regulations, and licensing requirements in their territory.
