Exhibit D lists the names, business addresses, and telephone numbers of all current Caring Transitions franchises as of December 31, 2024 and those franchisees that have signed a franchise agreement but were not yet operational at the end of the year. Exhibit E lists the name, city and state, and the current business telephone number (or, if unknown, the last known home telephone number) of every Caring Transitions franchisee who has had their franchise terminated, canceled, transferred, not renewed, or otherwise voluntarily or involuntarily ceased to do business under the franchise agreement or has not communicated with us within 10 weeks of the issuance of this disclosure document. If you buy this franchise, your contact information may be disclosed to other buyers when you leave the franchise system.

What This Means (2025 FDD)
According to the 2025 Caring Transitions Franchise Disclosure Document, Exhibit D lists the names, business addresses, and telephone numbers of all current Caring Transitions franchises as of December 31, 2024, including those that have signed a franchise agreement but were not yet operational. Additionally, Exhibit E lists information on franchisees who have left the system as of the fiscal year ending December 31, 2024, or who have not communicated with Caring Transitions within 10 weeks of the issuance of the disclosure document.
If a franchisee is not a limited liability business entity when signing the agreement, they have 90 days after signing to transfer their interest in the franchised business to such an entity. Furthermore, if a Cooperative for the geographical area in which the franchised business is located is established during the term of the agreement, the franchisee must immediately become a member.
Prospective franchisees should note these timelines and reporting requirements, as they impact the operational and legal structure of their Caring Transitions franchise. The FDD also states that the franchisor has the right to discontinue the operation of the website at any time in its business judgment.
