By signing the questionnaire, the franchisee represents that they have answered truthfully and acknowledge the importance of their responses. This acknowledgment is crucial for Canine Dimensions to ensure transparency and compliance with legal requirements during the franchise sales process. However, Canine Dimensions will not ask you to complete this form, and will disregard any answers from you, if you live or plan to operate your franchise in the states of California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, Wisconsin.
