As you know, Canine Dimensions Franchising, LLC (the "Franchisor") and you are preparing to enter into a franchise agreement (the "Franchise Agreement") for the establishment and operation of a Canine Dimensions Business (the "Franchised Business"). The purpose of this Questionnaire is to determine whether any statements or promises were made to you by employees or authorized representatives of the Franchisor, or by employees or authorized representatives of a broker acting on behalf of the Franchisor ("Broker") that have not been authorized, or that were not disclosed in the Disclosure Document or that may be untrue, inaccurate or misleading. The Franchisor, through the use of this document, desires to ascertain: (a) that the undersigned, individually and as a representative of any legal entity established to acquire the franchise rights, fully understands and comprehends that the purchase of a franchise is a business decision, complete with its associated risks, and (b) that you are not relying upon any oral statement, representations, promises or assurances during the negotiations for the purchase of the franchise which have not been authorized by Franchisor.

What This Means (2025 FDD)
According to Canine Dimensions's 2025 Franchise Disclosure Document, the franchisee's acknowledgment of reviewing the Franchise Agreement serves several critical purposes. Primarily, it aims to confirm that the franchisee understands the agreement's contents and recognizes that purchasing a franchise is a business decision involving inherent risks. This acknowledgment is documented through a questionnaire designed to reveal any unauthorized statements or promises made during the negotiation process that contradict the official Disclosure Document.
By signing the Franchisee Disclosure Acknowledgment Statement, the prospective Canine Dimensions franchisee confirms they are not relying on any verbal assurances or representations not explicitly authorized by Canine Dimensions. This protects Canine Dimensions from potential legal disputes arising from misunderstandings or misinterpretations of information provided outside the official documentation. The questionnaire also covers whether the franchisee has received, reviewed, and understood the Franchise Agreement and Disclosure Document, and whether they consulted with professional advisors regarding the benefits and risks of the franchise.
Furthermore, the acknowledgment underscores that the franchisee's success depends largely on their own skills, abilities, and various market factors, rather than solely on promises made by Canine Dimensions. The franchisee also confirms that they haven't entered into any binding agreements concerning the franchise before the formal signing of the Franchise Agreement. This comprehensive acknowledgment process is a standard practice in franchising, designed to ensure both parties have a clear understanding of the terms and risks involved, and to protect the franchisor from unwarranted claims.
