SMYA VENTURES LLC (the "Franchisor") and you are preparing to enter into a Franchise Agreement for the establishment and operation of a Buns on Fire restaurant (the "Franchised Business"). The purpose of this Questionnaire is to make sure you have received the information necessary and have had the opportunity to review and consider such information.

What This Means (2025 FDD)
According to the 2025 Buns On Fire Franchise Disclosure Document, Exhibit H is a questionnaire that prospective franchisees must review and respond to. The purpose of this questionnaire is to ensure that the franchisee has received and had the opportunity to review all necessary information before entering into a franchise agreement with Buns On Fire.
The questionnaire is separate from any communications a franchisee may have had with a transferring franchisee if they are purchasing an existing store. The questions within Exhibit H specifically relate to the information provided by SMYA Ventures LLC, the franchisor, and not any third-party communications.
Prospective franchisees in California should note that the California Addendum to the Franchise Agreement states that California franchisees should not complete Questions 9-15 of the Questionnaire annexed to the Franchise Agreement as Exhibit J. If they do, Buns On Fire will destroy and disregard it.
