You are preparing to enter into a BODY20® Franchise Agreement (the "Franchise Agreement") with BODY20 Franchisor LLC ("we" or "us"). The purpose of this Questionnaire is to confirm that you understand the terms of the contract and that no unauthorized statements or promises have been made to you. Please review each of the following questions and statements carefully and provide honest and complete responses to each.

What This Means (2025 FDD)
According to Body20's 2025 Franchise Disclosure Document, the compliance questionnaire that prospective franchisees must complete before signing the Franchise Agreement explicitly requires honest and complete responses to each question. The FDD states that the purpose of the questionnaire is to confirm the franchisee understands the terms of the contract and that no unauthorized statements or promises have been made.
The questionnaire is not required if the franchise is to be operated in, or the franchisee is a resident of, California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin. Furthermore, the acknowledgement should not be signed if the franchise is to be operated in, or the franchisee is a resident of, Maryland.
Body20 emphasizes the importance of the responses, stating that they will rely on the franchisee's representations. By signing the questionnaire, the franchisee confirms that they have responded truthfully to the questions.
