You are preparing to enter into a BODY20® Franchise Agreement (the "Franchise Agreement") with BODY20 Franchisor LLC ("we" or "us"). The purpose of this Questionnaire is to confirm that you understand the terms of the contract and that no unauthorized statements or promises have been made to you. Please review each of the following questions and statements carefully and provide honest and complete responses to each. In this Questionnaire, our "representatives" include our officers, directors, employees, agents, sales brokers, and/or any other representatives working on our behalf.

What This Means (2025 FDD)
According to the 2025 Body20 Franchise Disclosure Document, the purpose of the compliance questionnaire is to confirm that the franchisee understands the terms of the Franchise Agreement and that no unauthorized statements or promises have been made to them. This questionnaire is completed before signing the Franchise Agreement.
The questionnaire requires the franchisee to carefully review each question and statement and provide honest and complete responses. Body20 representatives include officers, directors, employees, agents, sales brokers, and any other representatives working on their behalf.
However, the questionnaire does not apply if the offer or sale of the franchise is subject to state franchise disclosure laws in California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, or Wisconsin. Additionally, the acknowledgement should not be signed if the franchise is to be operated in, or the franchisee is a resident of, Maryland. This suggests that Body20 aims to ensure franchisees in states without strong franchise laws are fully informed, while adhering to stricter regulations in states with franchise-specific protections.
By using this questionnaire, Body20 aims to mitigate potential disputes by ensuring franchisees acknowledge their understanding of the franchise terms and that their decision is based on the official documentation rather than unofficial promises. This practice is common in franchising to protect both the franchisor and franchisee by establishing a clear record of the franchisee's understanding prior to entering the agreement.
