What This Means (2025 FDD)
According to Bhc's 2025 Franchise Disclosure Document, the prospective franchisee must provide certain information as part of the Acknowledgment and Authorization for Background Check. This includes indicating whether they intend to have a co-franchisee, and if so, providing the names and estimated percentage of ownership for each prospective co-franchisee. Each prospective franchisee must complete and sign the Acknowledgment and Authorization for Background Check.
For individual applicants, Bhc requires their name, date and place of birth, address, home and cell phone numbers, and citizenship status. If not a U.S. citizen, the applicant must specify their citizenship. The applicant must also disclose any felony convictions or pending felony charges, and provide details if applicable. Information about whether they own or rent their residence and their length of residency is also required, including previous addresses if they have lived at their current residence for less than five years.
For entity applicants, Bhc requires the entity's name, date and place of formation, address, office phone number, and the names of officers, directors, shareholders, members, or partners. The document also requires information on the prospective franchisee's education and employment/business experience, including high school, college, and graduate school details, as well as company names, addresses, dates of employment, titles, number of employees supervised, duties and responsibilities, and type of business. Bhc also requires financial statement information from the prospective franchisee.
