What is contained in Exhibit A of the Beyond Juicery Eatery Telephone Number Assignment?

According to the 2025 Franchise Disclosure Document, Exhibit A to the Telephone Number Assignment agreement for Beyond Juicery Eatery contains the franchisee's telephone number. The Telephone Number Assignment agreement states that the franchisee assigns all rights, titles, and interests in the telephone numbers listed in Exhibit A to Beyond Juicery Eatery. This assignment becomes effective upon the expiration or termination of the Franchise Agreement without renewal.

Beyond Juicery Eatery is authorized to notify telephone companies and directory services of this assignment, which will be construed as the franchisee's cancellation and surrender of the numbers, allowing Beyond Juicery Eatery to reassign them. The agreement also stipulates that Beyond Juicery Eatery will provide notice of its acceptance of the telephone number assignment.

Furthermore, the franchisee is obligated to cooperate with Beyond Juicery Eatery in executing any necessary documents to effect the assignment. If the franchisee fails to sign these documents within two business days of notice, Beyond Juicery Eatery is appointed as the franchisee's attorney to sign on their behalf. All telephone charges, including directory advertising, will be prorated as of the assignment date, with the franchisee responsible for charges before the assignment and Beyond Juicery Eatery responsible for subsequent charges.
