What This Means (2025 FDD)
According to Basecamp Fitness's 2025 Franchise Disclosure Document, a client's participation in any Fitness Incentive Program requires them to review and agree to the terms and conditions specific to that program. These terms and conditions may vary from one Fitness Incentive Program to another. Healthy Contributions (HC) acts as the payment processor and program administrator for these programs offered by various Program Providers. By choosing to participate in any Fitness Incentive Program administered by HC, the client agrees to abide by HC's terms and conditions, which may be updated or amended over time.
As a client, you must provide HC with accurate data and information reflecting actual Program Member participation in the Fitness Incentive Program. This data needs to be updated correctly in the Healthy Contributions website by the 5th of every month for the preceding month. Clients are also expected to review monthly return reports data as soon as they are available and address any payment discrepancies found. Additionally, clients must promptly reimburse the Program Provider for any funds received based on inaccurate data submitted by the client or paid by mistake.
Basecamp Fitness clients should also be aware that HC may communicate updates, changes, enhancements, offers, and other pertinent information about Fitness Incentive Programs via email or other electronic means. These communications may include information concerning health plans or promotional advertising related to Fitness Incentive Programs and/or HC's services. Furthermore, clients may be required to demonstrate that they maintain liability insurance at levels required by the Program Provider. A Program Provider and/or HC may audit the client's participation in any Fitness Incentive Program, and the client is expected to provide full cooperation in connection with any such audit.
Failure to comply with these terms and conditions can lead to termination of the agreement. HC may terminate the agreement immediately if it determines that the client has repeatedly failed to abide by the terms or has intentionally provided inaccurate, false, or faulty data in connection with any Fitness Incentive Program. Therefore, it is crucial for clients to carefully review and adhere to all terms and conditions to maintain their participation in the Fitness Incentive Programs.
