As you know, Augusta Franchise LLC ("we" or "us"), and you are preparing to enter into a Franchise Agreement for the operation of Augusta Lawn Care Business (as defined in this Franchise Disclosure Document). The purpose of this Questionnaire is to determine whether any statements or promises were made to you that we have not authorized or that may be untrue, inaccurate or misleading, to be certain that you have been properly represented in this transaction, and to be certain that you understand the limitations on claims you may make by reason of the purchase and operation of your franchise. You cannot sign or date this Questionnaire the same day as the Receipt for the Franchise Disclosure Document but you must sign and date it the same day you sign the Franchise Agreement and pay your Initial Franchise Fee. Please review each of the following questions carefully and provide honest responses to each question. If you answer "No" to any of the questions below, please explain your answer in the table provided below.

What This Means (2025 FDD)
According to Augusta Lawn Care's 2025 Franchise Disclosure Document, you are required to sign a receipt acknowledging that you received the FDD. Exhibit I to the FDD includes detachable documents for this purpose. You must sign one copy and return it to Augusta Franchise LLC at 8115 Birch Bay Square, Suite 133 Blaine, Washington 98230, or via email to franchise@augustalawncareservices.com. A duplicate receipt is for your own records.
Furthermore, a questionnaire included in the FDD asks whether you signed a receipt for the Franchise Disclosure Document indicating the date you received it. This questionnaire is designed to confirm that you have been properly represented in the transaction and understand the limitations on claims you can make. However, you cannot sign or date this questionnaire on the same day you sign the receipt for the FDD; it must be signed and dated on the same day you sign the Franchise Agreement and pay your Initial Franchise Fee.
It is important to note that this requirement does not apply to California or Washington residents or franchisees, nor should the questionnaire be signed if the franchisee is a Maryland resident or if the franchised business will be located in Maryland. This receipt serves as an acknowledgment that Augusta Lawn Care provided you with the necessary information to make an informed decision, as mandated by franchise laws.
