Yes, according to the 2024 All States M.E.D. Franchise Disclosure Document, Exhibit H is the Franchisee Disclosure Questionnaire. Item 22 lists the contracts attached to the disclosure document, and Exhibit H is explicitly listed as the Franchisee Disclosure Questionnaire.
Exhibit H itself contains a questionnaire that All States M.E.D. franchisees must complete.
