We authorize the respective state agencies identified on Exhibit B to receive service of process for us in the particular state.
I have received a Franchise Disclosure Document dated April 26, 2024 including the following exhibits on the date listed below:
- B. State Agents for Service of Process

What This Means (2024 FDD)
According to All States M.E.D.'s 2024 Franchise Disclosure Document, the list of State Agents for Service of Process can be found in Exhibit B. All States M.E.D. authorizes the respective state agencies identified in Exhibit B to receive service of process for them in each particular state.
Exhibit B includes the names, addresses, and phone numbers for the relevant authorities in states that require franchise registration, such as California, Hawaii, Illinois, Indiana, Maryland, Michigan, Minnesota, New York, North Dakota, Rhode Island, South Dakota, Virginia, Washington, and Wisconsin. For example, in California, the agent for service of process is the Commissioner of Financial Protection and Innovation located in Los Angeles, California.
Prospective franchisees can use this list to understand who is authorized to accept legal notices on behalf of All States M.E.D. in their state. This is important in case there is ever a legal dispute or need to serve legal documents to the company. The exhibit ensures that there is a clear and official channel for legal communication, which is a standard practice in franchising to maintain transparency and legal compliance.
