AMIV FRANCHISE DISCLOSURE DOCUMENT 22 You must engage a medical director that you choose but that is approved by us in writing. We currently refer franchisees to some physicians that they can choose to affiliate with as their medical director. You may use another medical director approved by us. The medical director is responsible to review, oversee, and manage all patient bookings and patient care including care and services provided by any and all licensed healthcare professionals and staff who provide actual intravenous therapy and hydration services to be provided through the franchised business. Only licensed healthcare professionals may actually administer the intravenous therapies and hydration, such as registered nurses. You may not provide, control, or supervise the intravenous therapies or hydration services if you are not properly licensed and qualified. In states that have
adopted the corporate practice of medicine, there are additional restrictions and prohibitions. Because various federal and state laws govern the practice of medicine and the ownership of medical practices and healthcare businesses, it is critically important that a franchisee that is not a licensed healthcare professional does not engage in conduct or practices that are, or may appear to be, the practice of medicine. The medical director must be responsible for and offer all intravenous therapies and hydration services in accordance with all relevant laws and regulations. If, at any time, a franchise is not operated as required by relevant local licensing, practice, or layperson-influence rules, we will require the franchise to be transferred to an appropriately licensed medical professional with a current license, or we may terminate the franchise. A copy of the agreements offered by the physicians that we refer to our franchisees as of the date of this disclosure document are included in Exhibit I to this disclosure document; they include a management services agreement and practice equity holder agreement.
You must purchase all equipment, inventory, and all other items used in your franchised business in accordance with our specifications and guidelines to ensure the quality and uniformity of services in the AMIV franchise system.

The medical director must also be named within the franchisee's medical malpractice coverage policy, which must have a limit of at least $1,000,000, and list Advanced Mobile IV, LLC as an additional insured.
