1-800-GOT-JUNK? franchisee.
When you have completed the questionnaire, please sign the certification
that appears on the last page.
The overall purpose of the information collected by this questionnaire is to determine whether
any statements or promises were made to you by employees or representatives of 1-800-GOT-JUNK?
LLC that 1-800-GOT-JUNK?
LLC has not authorized, and that may be untrue, inaccurate or misleading.
With that purpose in mind, you will find the questions with regard to statements that may have been made
to you during the application process.
In addition, questions relating to statements made to you during the application process, you will
also find questions relating to the dates that certain documents (such as the Franchise Disclosure
Document or Franchise Agreement) were received, or dates on which payments were made.
When
purchasing a franchise, the timing of the receipt of documents, payments of franchise fees and other
events are very important.

What This Means (2025 FDD)
According to the 2025 1-800-GOT-JUNK? Franchise Disclosure Document, the Franchisee Disclosure Questionnaire and Certification can be found in Exhibit J. This exhibit includes a questionnaire that prospective franchisees must complete as they go through the process of becoming a 1-800-GOT-JUNK? franchisee.
The purpose of this questionnaire is to determine if any unauthorized statements or promises were made by 1-800-GOT-JUNK? employees or representatives during the application process. The questionnaire also asks about the dates certain documents, such as the Franchise Disclosure Document and Franchise Agreement, were received, and the dates on which payments were made. The timing of these events is important when purchasing a franchise.
After completing the questionnaire, the franchisee must sign the certification on the last page, representing that their answers are truthful. The completed Franchise Disclosure Questionnaire and Certification should be provided when signing the 1-800-GOT-JUNK? Agreement. However, the Franchisee Acknowledgement is not applicable to any franchise offer or sale involving any California resident or franchisee, as it violates California Corporations Code sections 31512 and 31512.1. Additionally, the questionnaire and certification should not be signed if the franchisee is a resident of Maryland or Washington, or if the business is to be operated in either of those states.
